Harmony Hospice has the finest health care providers with years of experience caring for patients with a terminal illness. To complement our hospice service, Harmony now has an innovative community based palliative care division. Harmony Hospice’s dedicated palliative interdisciplinary team cares for patients with a chronic or serious illness, but who aren’t yet ready for hospice care.

 

Our care includes:

· Keep patients safe and comfortable in their own homes

· Decrease unnecessary hospital and emergency room visits

· Provide services to patients who are still seeking curative treatment for a terminal/chronic illness

· Fill the gap for patients who do not yet meet hospice criteria but are still in need of additional support services

· Symptom Management for a serious illness

· Medication reconciliation

· Care coordination with the member’s primary care provider

· 24/7 access to services

· Community resource access and referral

· Current disease specific education

· Advance directive discussion and advice

CNA HOSPICE AIDES

Hospice Aides are the UNSUNG HEROES of Hospice Care.

We are seeking compassionate CNA Hospice Aides with a background in geriatrics and palliative care. Candidates must be able to work well as part of a team. As a hospice aide, you provide direct personal care that may incontinence care, assistance with hair, skin, oral hygiene, nails, and showers. and share observations and problems with the RN Case Manager and the IDG Team.

RN CASE MANAGERS

We are looking for nurses who want something more than a JOB, but their CALLING.

RN Case Managers are a part of an EXTRAORDINARY TEAM, which includes CNAs, Social Worker, and Spiritual Care Counselor for each patient.

As a case manager, you will create a care plan to manage the individual patient’s pain needs and provide education on end-of-life issues for the patient and the family.
