End-of-Life Doula Training: Participant Application

Please respond to the following questions with thoughtful reflection. Please do not respond with one or two-word answers. Admission to this program is generous, so you don’t need to worry that we will not select you. The purpose of this application is for you to reflect on your reasons for joining this training, what you would like to get from it, and how you will use the information and skills you develop. It also helps us to get to know you a bit better.

After we have reviewed your application, you will receive a formal invitation to join the Spring, 2024 cohort. Please check your calendar before applying to ensure that you will be able to attend all sessions of the training. We will meet Thursday evenings in person from 5:30-8:30pm beginning Feb. 1 and running through April 25, 2024. The exception will be that there is no meeting on March 28 due to religious observances.

CNA HOSPICE AIDES

Hospice Aides are the UNSUNG HEROES of Hospice Care.

We are seeking compassionate CNA Hospice Aides with a background in geriatrics and palliative care. Candidates must be able to work well as part of a team. As a hospice aide, you provide direct personal care that may incontinence care, assistance with hair, skin, oral hygiene, nails, and showers. and share observations and problems with the RN Case Manager and the IDG Team.

RN CASE MANAGERS

We are looking for nurses who want something more than a JOB, but their CALLING.

RN Case Managers are a part of an EXTRAORDINARY TEAM, which includes CNAs, Social Worker, and Spiritual Care Counselor for each patient.

As a case manager, you will create a care plan to manage the individual patient’s pain needs and provide education on end-of-life issues for the patient and the family.
